Here for You – Objective

Provide individuals an easy-to-use resource that gives them personally actionable information regarding the resources available for personal health, public health, and public safety.  

Premise: We have a system of services, public and private, that are in place to support the safety and wellbeing of our community and the individuals who comprise it. 

Problem: Lack of understanding about the utility, value, availability and accessibility of these services is resulting in their inefficient, uncoordinated and ineffective use in support of personal and community health. 

Solution: A resource that provides individuals easy access to need-to-know, situation specific, and actionable information related to:

  1. Understanding of personal and social health needs 
  2. Identifying the availability of health care, public health and public safety provisions that address those needs
  3. Identifying the accessibility of those provisions 

The Social Determinants of Homelessness
