Here for You – Objective
Provide individuals an easy-to-use resource that gives them personally actionable information regarding the resources available for personal health, public health, and public safety.
Premise: We have a system of services, public and private, that are in place to support the safety and wellbeing of our community and the individuals who comprise it.
Problem: Lack of understanding about the utility, value, availability and accessibility of these services is resulting in their inefficient, uncoordinated and ineffective use in support of personal and community health.
Solution: A resource that provides individuals easy access to need-to-know, situation specific, and actionable information related to:
- Understanding of personal and social health needs
- Identifying the availability of health care, public health and public safety provisions that address those needs
- Identifying the accessibility of those provisions
The Social Determinants of Homelessness
